Your first visit
We look forward to meeting with you. Please review the information below regarding how to prepare for your first appointment:
- Bring the referral provided by your general dentist or orthodontist.
- Bring a list of any medications you currently are taking.
- Please bring x-rays if you have them.
- Minors (under 18 years old) must have a parent or legal guardian present.
- If you have insurance, please bring signed and completed forms. You will be required to pay deductible and/or percentage not covered. If no insurance, payment is required on day of surgery.
- If you are unable to keep an appointment, please notify the office as soon as possible.
Please plan to arrive 15 minutes prior to your appointment. If possible, download and complete the attached Registration Form and bring it with you on the day of your appointment.
At Mountainside Oral & Maxillofacial Surgery, we value our patients' time and make every effort to keep our appointments running on-schedule. However, due to the nature of our office providing surgical treatment and, at times, emergency care, circumstances may arise causing a procedure to extend longer than expected. We appreciate your patience and understanding in these situations.
